Meet Dr. Sarah Adams
Dr. Sarah Adams is a board-certified family physician and the founder of Palm Valley Family Medicine, a concierge Direct Primary Care practice, serving Ponte Vedra, Nocatee, and the surrounding Northeast Florida communities.
She built her practice on a simple belief: exceptional healthcare starts with listening. After years of watching traditional medicine squeeze patients into fifteen-minute appointments, Dr. Adams set out to build something different. She implemented a model where physicians have the time to get to know their patients, and where patients have direct access to their physician when it really matters.
Dr. Adams combines evidence-based medicine with ample appointment time, proactive care, and genuine relationships to help her patients live healthier, longer lives. Her practice is built around prevention, longevity, hormone optimization, pediatrics, weight management, and comprehensive family medicine.
